June 2, 2022, Inspection at Spanish Trails Rehabilitation Suites
During a standard inspection at Spanish Trails Rehabilitation Suites on June 2nd, 2022, surveyors noted several violations of resident care. The facility was cited for not effectively managing residents’ pain, not taking care of residents’ personal hygiene, not monitoring residents’ weights, and more. During interviews with staff, surveyors found that the facility was understaffed. A nurse told the surveyors, “I have about 20 wounds to do today (wound care) and I won’t be able to do any because I’m on the floor from 6:00am to 6:00pm.”
March 17, 2022, Inspection at Spanish Trails Rehabilitation Suites
An inspection at the facility on March 17th, 2022, revealed that Spanish Trails Rehabilitation Suites failed to timely follow-up on complaints and report allegations of abuse to proper authorities. When reviewing records from the facility, inspectors found a grievance report dated February 2nd, 2022, that detailed a CNA using force to clean the resident during a diaper change and the CNA resting in bed with the resident. The DON confirmed that this grievance was not investigated by the facility until February 20th.
