Morris, Sharon E. E/O vs. Dolphin Pointe Health Care
Allegations: In August of 2024, Tammy Morris, representing the estate of Sharon Morris, sued Dolphin Pointe Health Care for nursing home negligence. Allegedly, while she was a resident of the facility, Sharon Morris suffered from neglect resulting in several injuries. The case’s complaint lists her injuries as a left shoulder dislocation, moisture associated skin damage (MASD) to her thighs and buttocks, a necrotic pressure ulcer on her sacrum, pneumonia, and sepsis. Additionally, upon admission into a hospital from Dolphin Pointe, Sharon Morris tested positive for MDMA, or ecstasy, in her system. Ms. Morris was unable to recover from her many injuries and passed away in hospice care.

Bean, Paula E/O vs. University Center for Nursing and Healing
Allegations: In July of 2024, a nursing home neglect case was filed against University Center for Nursing and Healing. Gregory Bean, the personal representative of Paula Bean’s estate, alleged that Ms. Bean’s preexisting bed sore severely deteriorated under the facility’s care. Ms. Bean was admitted to University Center with a stage two hospital-acquired pressure ulcer. University Center was aware of the wound but failed to provide appropriate wound care and offloading to promote wound healing. Ms. Bean’s stage two wound deteriorated into an infected, tunneling, stage four wound. As a result, Ms. Bean suffered an untimely death.
River City Nursing and Rehab Center
River City Nursing and Rehab Center, known under the legal business name of First Coast Health Ventures, LLC., is a 116-bed nursing home in Duval County. The facility is located at 15480 Max Leggett Parkway in Jacksonville, Florida.
Turner, Valvetta E/O vs. River City Nursing and Rehab Center
Allegations: In June of 2024, Antoinette Cummings, representing the estate of Valvetta Turner, filed a nursing home negligence lawsuit against River City Nursing and Rehab Center. During Ms. Turner’s residency at the facility, she suffered a fall resulting in a femur fracture. The claim alleges that Ms. Turner’s health deteriorated quickly after her injury, resulting in her untimely death.

Fouraker Hills Rehab and Nursing Center, affiliated with Citadel Care Centers, is located at 1650 Fouraker Road in Jacksonville, Florida. This for-profit facility is also known under the legal business name of West Jacksonville Care Acquisition, LLC.
Cepeda, Theresa E/O vs. Fouraker Hills Rehab and Nursing Center
Allegations: Julio Cepeda, representing the estate of his late wife, Theresa Cepeda, filed a lawsuit against Fouraker Hills Rehab and Nursing Center in May of 2024. The case alleges that while Mrs. Cepeda was a resident of the facility, she suffered from progressive swallowing difficulties. Staff did not adequately address her problems swallowing, which led to a choking event resulting in aspiration pneumonia and severe encephalopathy. Food content was found in her airway at the hospital. Mrs. Cepeda was transferred into hospice care where she passed away from her injuries.

Lakeside Center for Rehabilitation and Healing is a 122-bed facility located at 11411 Armsdale Road in Jacksonville, Florida. The facility’s legal business name is Lakeside SNF Operations, LLC.
Crenshaw, Marsha E/O vs. Lakeside Center for Rehabilitation and Healing
Allegations: Jeremy Doudna, the personal representative of Marsha Crenshaw’s estate, filed a nursing home neglect claim against Lakeside Center for Rehabilitation and Healing in April of 2024.
