Colonna, John V. E/O vs. Southwestern Nursing and Rehabilitation Center
Allegations: On July 23rd, 2021, Mr. Colonna was transported from the facility to an outside appointment in a facility-owned van. Mr. Colonna’s wheelchair was not properly anchored to the transport van’s floor when staff loaded him in. During transport, the van was driven at unsafe speeds, causing Mr. Colonna’s wheelchair to topple over. He hit his head on the van’s floor, ultimately causing his untimely death.
Deficiencies at Southwestern Nursing and Rehabilitation Center for Resident Care Violations
June 30th, 2022, Inspection at Southwestern Nursing and Rehabilitation Center
During a complaint inspection at Southwestern Nursing and Rehabilitation Center, it was noted that staff failed to provide appropriate wound care and failed to prevent new bedsores from developing. Review of the patient’s record showed that the patient had developed an unstageable pressure ulcer (also known as a bedsore) on the right heel. The record states that the severity of the wound was discovered in February of 2022. Further review revealed that staff failed to provide the resident with any wound care until May of 2022.
April 4th, 2022, Inspection at Southwestern Nursing and Rehabilitation Center
Staff at Southwestern Nursing and Rehabilitation Center were found to have failed to properly assess, treat, and document changes in a resident’s condition. Review of the resident’s clinical record showed that they were being treated for an infection with an oral antibiotic. The resident sent to the hospital after being found unresponsive with an elevated heart rate and low blood pressure. Review of the nurses’ progress notes for the resident contained no notes about the hospitalization or change in condition.
