Deficiencies at Fayette Health and Rehabilitation Center for Resident Care Violations
March 11, 2019 Inspection at Fayette Health and Rehabilitation Center
In March of 2019, Fayette Health and Rehab Center underwent a required health inspection that uncovered unsanitary conditions in the facility’s food service. Boxes of chemicals, including detergents, were located in the same space as foods, and residual dried food debris was found on baking pans that could contaminate other food. In addition, fish such as tilapia were served at 100 degrees Fahrenheit internal temperature, when the USDA guidelines specify 145 degrees as the safest minimum temperature for fish and shellfish. Fourth floor residents some distance from the main kitchen area were being served the tilapia for some time after it had been at its warmest.
Additionally, some residents were not fed at scheduled times, and another resident who required frequent dressing changes to a wound did not receive regular changes, sometimes going as long as a whole day without wound attention. If a wound like a bed sore goes untreated it could lead to an infection and possible sepsis. A different resident experienced sudden and dramatic weight loss of more than 40 pounds in one week, and this change in health was not reported to the resident’s physician.
July 17, 2020 Inspection at Fayette Health and Rehabilitation Center
In the facility’s July 2020 inspection, Fayette again received health-related citation notification, this time as a result of its practices surrounding emergency equipment. The facility’s automated external defibrillator (AED)—a system that can detect an irregular cardiac rhythm and deliver a shock to restore the heart to a functional beat in case of an emergency cardiac event—was not used in the case of an unresponsive resident. When EMTs questioned why the AED was not used, the staff at Fayette Health and Rehabilitation Center explained that the AED was broken. The facility was issued a statement of deficiency for its lack of a company-wide system to monitor and appropriately replace the batteries and function of its AED systems within the building.
