June 9, 2021 Inspection at Birch Manor Center for Rehabilitation and Healthcare
During a record review, it was determined that the facility had failed to protect a resident from suffering a fall. During a diaper change, the resident was instructed to stand up and hold onto the dresser in their bedroom. The resident lost balance and fell, injuring their back and legs.
March 4, 2021 Inspection at Birch Manor Center for Rehabilitation and Healthcare
Staff at Birch Manor Center for Rehabilitation and Healthcare failed to honor a resident’s Maryland Medical Orders for Life-Sustaining Treatment (MOLST) order. The resident and their physician had previously made it known on the resident’s MOLST order that the resident was not to be resuscitated. A nurse found the resident unconscious and unnecessarily started CPR after misunderstanding the resident’s MOLST order.
December 23, 2020 Inspection at Birch Manor Center for Rehabilitation and Healthcare
Based on medical record review and interview, Birch Manor Center failed to document if COVID-19 testing was offered and completed. There was no existing record of test results in multiple residents’ medical records. After this inspection, a review of testing documentation was completed with the facility’s staff.
August 3, 2018 Inspection at Birch Manor Center for Rehabilitation and Healthcare
This inspection revealed that the facility failed to timely report instances of abuse and/or neglect to the State Survey Agency and Office of Health Care Quality. A resident had sustained a bruise on his upper left eye and was taken to the emergency room for it. When asked if this incident was reported, staff said no. Furthermore, the facility failed to investigate the cause of the bruising.
April 7, 2017 Inspection at Birch Manor Center for Rehabilitation and Healthcare
Based on medical record review and staff interview, this inspection found that staff at Birch Manor Center for Rehabilitation and Healthcare failed to alert a resident’s significant changes in health to family members and physicians. The incident began when a resident began complaining of shortness of breath. After this initial complaint, nurses began the resident on oxygen therapy to alleviate the issue. The next documentation in the resident’s medical record stated that the resident could no longer swallow and was not able to take medication or eat meals. A chest x-ray completed at the facility revealed the resident was suffering from aspiration pneumonia. After being transferred to a hospital, the resident was placed on a ventilator.
