Even though he had hit his head during the fall and was noted to be in pain, Mr. Minutolo was allegedly not sent from Encompass to the hospital until approximately 24 hours after the fall. Mr. Minutolo was diagnosed with a subdural hematoma, or brain bleed. Mr. Minutolo underwent a craniotomy but was ultimately unable to recover and passed away.

March 18th, 2022, Inspection at Encompass Health Rehabilitation Hospital of Largo
An Immediate Jeopardy (IJ) was implemented by the Florida Agency for Health Care Administration (AHCA) during a complaint inspection at the facility in March of 2022. An IJ is a situation in which the facility’s noncompliance with regulations has caused or is likely to cause serious injury or death to a patient. It is documented that a female resident, referred to as Patient #1 in records, reported an incident of sexual assault to staff. Patient #1 alleged that a man (Patient #2) had entered her room and sexually assaulted her. For four days after the incident, staff kept Patient #1 in her room that was adjacent to Patient #2’s. A physical exam to check for injuries was not completed until seven days after the assault. Additionally, the alleged predator, Patient #2, was not placed under any additional monitoring.
