Mr. Leonard suffered two preventable falls within the facility which resulted in a subdural hematoma (brain bleed), nasal bone fractures, loose front teeth, and abrasions.
Lucas, Anna E/O vs. Alden Lakeland Rehabilitation and Health Care Center
Allegations: Steven Lucas, the administrator of Anna Lucas’ estate, filed a wrongful death claim against Alden Lakeland. According to the case’s complaint, Ms. Lucas suffered from the lack of proper care in the facility, resulting in a severe pressure ulcer. Ms. Lucas’ wound contributed to her untimely passing.

June 8th, 2023, Inspection at Alden Lakeland Rehabilitation and Health Care Center
During an inspection at Alden Lakeland on June 8th, 2023, surveyors documented that the facility had failed to adhere to pressure ulcer prevention measures (such as turning and repositioning residents in bed). This failure resulted in five residents suffering from the development and deterioration of pressure ulcers (also known as bed sores or decubitus ulcers).
February 4th, 2023, Inspection at Alden Lakeland Rehabilitation and Health Care Center
A complaint inspection occurred in February at Alden Lakeland following a resident complaint about missing lottery tickets. The resident, identified as R3 in the Centers for Medicare & Medicaid Services (CMS) report, had purchased three lottery tickets, which then went missing in the facility. When R3 reported the missing tickets to a nurse, identified as V7, V7 laughed at R3 and said that he hadn’t won anything. R3 became upset. The administrator of the facility stated to CMS surveyors that it was inappropriate for V7 to joke at R3’s expense.
