Recent citations for Dade City Health and Rehabilitation Center include findings for resident protection, emergency care, supervision, and staff competency. Some were classified as “immediate jeopardy,” the most serious category of nursing home deficiency.
Abuse, Neglect, and Resident Protection
A December 17, 2025, complaint inspection resulted in four immediate-jeopardy deficiencies. Inspectors found failures involving protection from:

The October 2023 inspection also identified an immediate-jeopardy deficiency involving accident hazards and adequate supervision. Inspectors determined that the facility failed to meet federal requirements intended to prevent avoidable accidents. Supervision concerns resurfaced during a July 2025 complaint inspection. That finding was less severe. Inspectors identified the potential for more than minimal harm, not immediate jeopardy.
