During his time at the nursing home, Mr. Lowery suffered multiple preventable falls. One such fall resulted in a subdural hematoma (also known as a brain bleed), which led to Mr. Lowery’s death.

Allegations: Mr. Clark was a known fall-risk with a history of falls. While residing in Crystal River Health and Rehabilitation Center, Mr. Clark suffered multiple preventable falls. As a result of one of his falls, he suffered a right hip fracture. Mr. Clark passed away due to complications regarding his hip fracture.
Gatling, Loretta C. E/O vs. Crystal River Health and Rehabilitation Center
Allegations: Ms. Gatling was admitted to Crystal River Health and Rehabilitation Center for rehabilitation. During her stay at the facility, she suffered a fall which resulted in a left femur fracture. Ms. Gatling’s health significantly declined due to the injury, and she passed away shortly after. The estate alleged wrongful death allegations.

February 10, 2022, Inspection at Crystal River Health and Rehabilitation Center
Crystal River Health and Rehabilitation Center failed to submit reports of alleged abuse to the Agency for Health Care Administration (AHCA). Review of facility records showed that three residents had made allegations of abuse and neglect to staff. These allegations were not investigated or reported to the proper authorities.
- Resident #10 reported to the facility that a certified nursing assistant (CNA) had verbally abused him.
- Resident #69 reported that he was told by staff to wet himself. He also felt that he was not receiving enough water.
- Resident #59’s daughter made a report after the resident was allegedly locked in a shower by staff.
September 30, 2020, Inspection at Crystal River Health and Rehabilitation Center
Based on interview and record review, staff at Crystal River Health and Rehabilitation Center failed to provide hygiene care to a resident. During an interview with a certified nursing assistant (CNA), surveyors were told that staff were too busy to keep all residents well groomed.
October 18, 2019, Inspection at Crystal River Health and Rehabilitation Center
This inspection at the facility revealed that the facility failed to implement an effective pest control program to ensure the facility is free of pests. During a tour of the nursing home, surveyors were informed of a cockroach infestation. Cockroaches were seen in resident rooms, in the medication room, and at the nurses’ station.
