July 18th, 2023, Inspection at Community Care Nursing Center
Based on interviews and record review, it was determined that staff at Community Care Nursing Center failed to protect residents from physical abuse. Residents with a history of violence and verbal aggression were not appropriately monitored to prevent abuse from occurring. During the inspection on 7/18/23, a resident informed the staff and the inspector of a fight happening between two other residents. Resident #2 had punched Resident #4 after Resident #4 had choked Resident #2. Record review showed that these residents had a history of altercations with one another, sometimes resulting in hospitalizations.
April 16th, 2023, Inspection at Community Care Nursing Center
A complaint inspection at Community Care Nursing Center revealed that facility staff failed to prevent the physical abuse of three residents by other residents. The residents involved were noted to have had altercations in the past. Incidents included a resident being slapped in the face, another resident being tussled to the floor, and a third resident being hit on the hand.
