James Long, Mrs. Herrera’s surviving spouse, alleges that during her residency at Jackson Place Nursing and Rehabilitation Center, she suffered respiratory failure, sepsis, and severe dehydration. Mr. Long alleges that because of the facility’s negligence, Mrs. Herrera suffered bodily injury and subsequently passed away on March 10, 2021.
Briel, Eulalia E/O vs. Jackson Plaza Nursing and Rehabilitation Center
Leslie Reyes, on the behalf of Mrs. Briel and her surviving spouse, Pedro Briel, filed suit against Jackson Plaza Nursing and Rehabilitation Center for failing to prevent the wrongful death of Mrs. Briel. Ms. Reyes alleges that the facility failed to prevent Mrs. Briel from developing bed sores. Ms. Consuelo-Burdie’s surviving children, led by her daughter Maura Krash, filed a lawsuit against Jackson Plaza Nursing and Rehabilitation Center after her death on April 24, 2019. Ms. Conseulo-Burdie’s death allegedly resulted from the facility’s failure to prevent and treat her respiratory failure.

Based on interviews and record review, it was determined that the facility failed to properly assess a resident’s Advance Directive upon his passing and did not contact the representative listed on the Advance Directive to notify her of the resident’s death.
December 12, 2019, Inspection at Jackson Plaza Nursing and Rehabilitation Center
An unannounced inspection at Jackson Plaza Nursing and Rehabilitation Center found that the facility failed to implement fall prevention measures for residents who were fall-risks. Inspectors noted that there were no floor mats next to residents’ beds and no bed rails. Record review showed that one of the residents without floor mats had fallen out of her bed just a month before.
