Deficiencies at The Rehabilitation Center of the Palm Beaches for Resident Care Violations
August 4th, 2022, Inspection at The Rehabilitation Center of the Palm Beaches
During a health inspection at The Rehabilitation Center of the Palm Beaches in August of 2022, surveyors conversed with a resident about an allergic reaction rash that he was dealing with. Surveyors observed that the resident had a general body rash with multiple scabs and the resident’s medical records noted that the rash continued onto the resident’s perianal area. Based on review of the facility’s records and notes, it was determined that the facility failed to assess and accurately reassess the efficacy of interventions for the rash. Nurses had been providing the same treatment to no avail for a month without trying another course of treatment.
July 1st, 2021, Inspection at The Rehabilitation Center of the Palm Beaches
A resident at risk for aspiration due to his medical diagnoses was left unsupervised while eating breakfast. This incident occurred during the Centers for Medicare & Medicaid Services (CMS) July 1st, 2021, inspection and was observed by a surveyor. The surveyor watched as the resident tried to consume “an estimated amount (of food) the size of a golf ball…The call bell was not noted within reach.” The facility’s care plan for the resident was reviewed with staff and stated that the resident needed to be observed while eating for signs of aspiration. Aspiration can occur when food or fluid is breathed into the lungs instead of being swallowed and can result in a pneumonia infection, choking, or even death.
March 9th, 2020, Inspection at The Rehabilitation Center of the Palm Beaches
A complaint inspection occurred at The Rehabilitation Center of the Palm Beaches in March of 2020 after a resident’s family member became concerned that the facility’s staff were not listening to their concerns. The family member’s loved one had lost almost 10 pounds in one month in the facility. On 2/1/20, the resident weighed 137 pounds and on 3/9/20, the resident weighed 127.5 pounds. It was determined that the facility had failed to provide the resident with appropriate nutritional care and intervene with a Registered Dietician.
October 10th, 2019, Inspection at The Rehabilitation Center of the Palm Beaches
During an inspection at the facility, surveyors overheard a Certified Nursing Assistant (CNA) telling a resident to “Shut up.” When surveyors asked the CNA to repeat what she had said to the resident she stated, “I am not going to lie, I told her to shut up.” The resident then informed the surveyors that staff frequently verbally abuse her.
