April 7th, 2022, Inspection at The Terrace at Bishop’s Glen
During this inspection at the facility, it was found that staff were not aware of the state’s requirement to have a registered nurse (RN) on duty eight hours a day. While reviewing the facility’s time sheets, it was determined that the facility went without an RN on duty for multiple days. Additionally, inspectors cited the facility due to multiple residents not receiving help maintaining their hygiene. The residents in question were unable to maintain their personal hygiene on their own.

According to this inspection at the nursing home, staff failed to notify the guardians of two residents that were transferred to the hospital.

Montgomery, Mabel May E/O vs. Terrace Healthcare and Rehabilitation Center, LLC.:
The personal representative of Ms. Montgomery’s estate, Mary Strawder, filed a lawsuit against Terrace Healthcare and Rehabilitation Center for nursing home negligence. The complaint alleges that while she was a resident of the facility, Ms. Montgomery fell multiple times, resulting in multiple fractures to her lower extremities (legs and ankles). As she was not a candidate for surgery, Ms. Montgomery was provided pain medications until her untimely passing.
Wollenberger, Eva E/O vs. Terrace Healthcare and Rehabilitation Center, LLC.:
Tina Wollenberger, acting as the representative of Eva Wollenberger’s estate, sued Terrace Healthcare and Rehabilitation Center for nursing home negligence and wrongful death. Allegedly, while under the care of the nursing facility, Eva suffered from the development of multiple bedsores, infections, and weight loss. Prior to her passing, she was transferred to the hospital for a stage four, bone-deep, sacral bedsore. The bedsore had become infected, and Ms. Wollenberger had gone into septic shock. After a debridement surgery of the wound, she was sent to hospice care. A few days later, she passed away from her injuries and infections.

Inspectors found that the facility had failed to notify a resident’s representative of an allegation of abuse that the resident had made.
January 21st, 2022, Inspection at Terrace Healthcare and Rehabilitation Center
Based on observations, interviews, and record review, inspectors determined that a resident was not receiving appropriate wound care. According to the resident’s records, the resident suffered from a stage two bedsore on their sacrum. While interviewing the resident, inspectors were informed that the wound dressing had not been changed in quite some time. Upon examination, inspectors could see serous fluid leaking from the dressing.

Camhi, Judith E. E/O vs. Terrace of Delray Beach, LLC.
David Camhi, one of Judith Camhi’s surviving sons, filed a lawsuit against The Terrace of Delray Beach Nursing and Rehabilitation Center for nursing home negligence. According to the case’s complaint, Ms. Camhi allegedly suffered from severe neglect while she was residing at the facility. Due to this neglect, she developed multiple bedsores and a sepsis infection. Ms. Camhi passed away as a result of her injuries.
Cantor, Philip vs. Terrace of Delray Beach, LLC.
Philip Cantor’s son and Power of Attorney, Steven Cantor, filed a lawsuit on his behalf against The Terrace of Delray Beach Nursing and Rehabilitation Center. The case alleges that Philip suffered from skin breakdown resulting in bedsores while in the facility.

During this inspection at the facility, it was found that a resident suffered an unexplained hip dislocation. Furthermore, the facility failed to investigate and report the incident to the proper state authorities.
February 24th, 2021, Inspection at The Terrace of Delray Beach Nursing and Rehabilitation Center
Based on observations made during this inspection at the facility, it was determined that The Terrace of Delray Beach Nursing and Rehabilitation Center failed to provide a comfortable, safe, and clean environment for their residents. Rooms were missing call lights, some bathroom lights were not working, there were cracks seen in the floors, etc.

Based on observations and interviews at the facility, inspectors determined that one of the residents at The Terrace of Hialeah was not receiving help maintaining her personal hygiene.
September 13th, 2019, Inspection at The Terrace of Hialeah
During an interview with a resident at The Terrace of Hialeah, the resident’s roommate began screaming at her and using foul language towards her. The resident explained to the inspector that her roommate verbally abuses her a lot, and that she has informed multiple staff members about the abuse, but nothing had been done to help the situation. The facility failed to report the abuse to the appropriate authorities until after this September 2019 inspection.

Maier, Kenneth E/O vs. Terrace of Jacksonville, LLC.:
Kenneth Maier’s son, Christopher Maier, acting as personal representative of his father’s estate, sued The Terrace of Jacksonville following Kenneth Maier’s untimely death. Allegedly, Kenneth Maier was sent to the hospital from The Terrace of Jacksonville with aspiration pneumonia, severe dehydration, malnutrition, and two unstageable bedsores. Mr. Maier passed away shortly after being put on hospice.
Makichuk, Linda L. E/O vs. Terrace of Jacksonville, LLC.:
Cynthia Oglesby, Ms. Makichuk’s daughter, filed a lawsuit against The Terrace of Jacksonville for wrongful death. The complaint alleges that while in the care of the nursing home, Ms. Makichuk suffered multiple falls. As a result of her final fall, she suffered a subdural hematoma, also known as a brain bleed. Ms. Makichuk was not taken to the hospital until several hours after the fall. Ms. Makichuk passed away in hospice care.
Deficiencies at The Terrace of Jacksonville for Resident Care Violations:
February 18th, 2021, Inspection at The Terrace of Jacksonville
It was noted during this inspection at the facility that staff had failed to prevent residents from suffering preventable falls. Based on interviews and record review, inspectors were made aware that a resident had fallen in the shower while under the watch of a CNA (certified nursing assistant). The CNA made no attempt to prevent the fall or catch the resident mid-fall. As a result of the fall, the resident suffered a left wrist fracture.
February 14th, 2019, Inspection at The Terrace of Jacksonville
The Terrace of Jacksonville was cited for failure to use skin breakdown prevention tools for a resident at risk for developing pressure ulcers. The resident’s chart stated that they either needed to have heel protectors or a pillow under their heels to offload pressure. Inspectors observed that neither of these prevention tools were being used during the time of the inspection.

The Terrace of Kissimmee is a 120-bed nursing home in Osceola County. It can be found at 221 Park Place Blvd. in Kissimmee, Florida. February 16th, 2023, Inspection at The Terrace of Kissimmee
During this inspection at The Terrace of Kissimmee, it was found that the facility had failed to investigate and report two allegations of abuse/neglect to the Agency for Health Care Administration (AHCA) within the required time period of five days.
May 20th, 2021, Inspection at The Terrace of Kissimmee
During this inspection at the facility, an inspector witnessed a resident fall. The resident hit their head on the floor and subsequently bled from a head laceration. The resident was sent to the hospital and diagnosed with several compression fractures as a result of the fall. At the time of the fall, the resident was unsupervised.
April 8th, 2021, Inspection at The Terrace of Kissimmee
An inspection occurred at The Terrace of Kissimmee following a complaint made by a resident’s family member. The family member, who was Power of Attorney (POA) over the resident, had not been informed of the resident’s outside doctor’s appointment. The resident was unable to make decisions for himself, so it was imperative that the resident’s POA be involved in all medical appointments.

August 2nd, 2022, Inspection at The Terrace of St. Cloud
An inspection at The Terrace of St. Cloud revealed that staff had not included a resident’s fall in their records. The resident had suffered a fall resulting in compression fractures in their lower spine and fractures of the right tibia and fibula. The resident’s hospitalization following the fall was included in records, but the fall itself was not.
December 9th, 2021, Inspection at The Terrace of St. Cloud
It was found that staff at The Terrace of St. Cloud failed to follow physician’s orders regarding a resident’s wound care. Per physician’s orders, the resident’s wound dressing was to be changed twice a day to promote healing. At the time of this inspection at the facility, the resident’s dressing had not been changed for two days.
March 5th, 2020, Inspection at The Terrace of St. Cloud
Based on observations, interviews, and review of medical records, inspectors determined that staff at The Terrace of St. Cloud failed to prevent the deterioration of a resident’s bedsores.
Terraces of Lake Worth Care Center Deficiencies
Terraces of Lake Worth Care Center, also known as Terraces Facility, Inc., is a 99-bed nursing facility in Palm Beach County. The facility can be found at 1711 6th Ave. S. in Lake Worth, Florida. At the time of this post, the facility has been rated five out of five stars by Medicare.

During this inspection at Terraces of Lake Worth Care Center, it was found that the facility had failed to complete care plans for twelve residents within seven days of their admittance to the facility.

While touring the Terraces of Lake Worth Care Center during this December 2019 inspection, an inspector found that the emergency call light system in the community shower room was not working.

The Terrace at Courtenay Springs, also known as Courtenay Springs Village, is an 80-bed nursing facility. It is located at 1100 S. Courtenay Parkway in Merritt Island, Florida. At the time of this post, the facility is rated two out of five stars by Medicare. Additionally, the facility has a “consumer alert” on its Medicare Care Compare page due to allegations of abuse.
Deficiencies at The Terrace at Courtenay Springs for Resident Care Violations:
November 9th, 2022, Inspection at The Terrace at Courtenay Springs
A verbal argument occurred between a resident and a CNA at the facility. The resident had expressed they were upset that the CNA was on her cellphone all the time. After yelling at the resident, the CNA smacked the resident’s hat off his head. During the 11/9/22 inspection at The Terrace at Courtenay Springs, inspectors found that a thorough investigation of the incident and 5-day report were not submitted to the appropriate state authorities.
September 24th, 2021, Inspection at The Terrace at Courtenay Springs
Based on interviews and record reviews, inspectors determined that staff at The Terrace at Courtenay Springs failed to provide basic life saving measures, including CPR, to a resident. The resident did not have a Do-Not-Resuscitate order. The resident passed away as a result of the lack of life saving procedures.

Based on observations, inspectors noted that staff at The Terrace at Courtenay Springs failed to properly clean a resident suffering from incontinence.
