Allegations: The personal representative of the estate of Ms. Roberts filed a lawsuit against Winter Haven Health and Rehabilitation Center after her passing. While a resident of the facility, Ms. Roberts developed a stage four pressure ulcer (bedsore) on her sacrum. Staff also failed to prevent Ms. Roberts from suffering falls in the facility which resulted in a head injury. Furthermore, the facility failed to prevent and treat a urinary tract infection (UTI), which led to Ms. Roberts suffering from urosepsis and septic shock. Ms. Roberts was hospitalized multiple times from injuries and infections caused by the negligence of Winter Haven Health and Rehabilitation Center.

Allegations: A lawsuit was filed against Winter Haven Health and Rehabilitation Center after Mr. Tomlinson suffered an untimely death as a result of the facility’s neglect. Allegations: The executor of the estate of Ms. Hughes sued Winter Haven Health and Rehabilitation Center for nursing home negligence and abuse. Ms. Hughes suffered from weight loss and skin integrity issues while she was a resident of the facility. Due to preventable falls in the facility, Ms. Hughes suffered from left and right foot fractures. She also developed severe bruising to her forehead, knees, and left thigh. Based on observations and interviews, the facility failed to provide a comfortable and home-like environment to residents. Inspectors learned that the washing machines at the facility were often out-of-order, which led to a shortage of clean clothes and linens. Residents described how they were often given pillowcases to dry off with after a shower or bed bath.
