Ms. Athey alleges that during Ms. Oliver’s stay at the facility, she was severely neglected by staff. This negligence led to the development of several urinary tract infections (UTIs) and sepsis. Due to a delay in treatment, Ms. Oliver passed away from the infections.

Allegations: Daisy Perry sued Hidden Valley Center for the negligent care she received while under their care. According to the case’s complaint, Ms. Perry suffered several preventable falls, a UTI, dehydration, and a C. Diff infection as a result of the facility’s negligence.
Shroyer, Alberta J. E/O vs. Hidden Valley Center
Allegations: The Estate of Alberta J. Shroyer, represented by Marsha Bishop, filed suit against Hidden Valley Center for nursing home negligence and wrongful death. During this inspection at Hidden Valley Center, it was found that staff had failed to ensure that Resident #44’s accident care plan was implemented. Resident #44 had a history of falls and was a fall-risk. A care plan was initiated in July of 2022 to implement fall prevention measures but was not being followed on the date of this inspection.
May 16th, 2023, Inspection at Hidden Valley Center
This complaint inspection at Hidden Valley Center resulted in a citation for failing to timely report (within 5 days) suspected abuse, neglect, or theft to the proper authorities. According to the resident involved, she suffered an instance of verbal abuse from a certified nursing assistant (CNA) in the morning of 4/6/23. The resident reported the incident to the facility’s social worker on the same day, who then alerted the facility’s administrator. However, by 5/16/23, over a month after the incident, the facility had still not reported the allegation to the State

Based on observation and interview, inspectors determined that Hidden Valley Center had failed to ensure that bathroom call devices were accessible to residents. The pull cords connected to the call devices were too short for residents to reach if they had fallen and couldn’t get up.
