Deficiencies at Hillcrest Center for Resident Care Violations
November 14th, 2022, Inspection at Hillcrest Center
This inspection at Hillcrest Center revealed that a resident, Resident R22, acquired a stage four bedsore on his left upper thigh. This wound was determined to have developed “in-house,” so while Resident R22 was in Hillcrest Center. Resident R22 had originally been admitted to Hillcrest Center with a bedsore on his sacrum that he had acquired elsewhere. Resident R22’s admission orders documented that Resident R22 was supposed to be provided with intensive wound care. His care plan stated that he was at risk of skin breakdown and needed to have frequent skin checks. A review of the nurses’ notes for Resident R22 suggests that nurses did not find his upper thigh bedsore until it was already a stage four wound.
November 16th, 2021, Inspection at Hillcrest Center
Based on interviews with staff and residents, review of records, and review of the facility’s policies, it was determined that Hillcrest Center failed to adequately supervise a resident and failed to protect that resident from physical abuse by another resident. Resident R93 who was diagnosed with dementia, had been frequently wandering in the facility and rummaging through other residents’ belongings. No extra supervision was provided to Resident R93 even with his increasing behaviors. Review of the facility’s RMS Event Summary Report documented that on March 10th, 2021, Resident R93 was seen by staff exiting another resident’s room. Resident R93 had a laceration on his right arm and three small hematomas on his forehead. 911 was called. In the meantime, staff went into the room belonging to Resident R565. When staff inquired about if he had caused Resident R93’s injuries, Resident R565 reportedly said, “Yup, I sure did. He came into my room and was going through all my stuff.” It was only after this incident that Resident R93 was placed on one-to-one supervision with a staff member.
August 8th, 2019, Inspection at Hillcrest Center
During this inspection at Hillcrest Center, it was found that nursing staff had failed to implement an ambulation program for at least one of the residents. Resident R130 had been evaluated by the facility’s physical therapist concerning the resident’s mobility status. The physical therapist instructed Hillcrest Center’s nursing staff to implement a restorative ambulation plan of care for Resident R130. The physical therapist expected nursing staff to assist Resident R130 to ambulate 150 feet daily. However, upon review of the resident’s clinical record revealed that there was no documented evidence of nursing staff providing this care.
