Sepelyak, Rosie M. E/O vs. Oakmont Center for Nursing & Rehabilitation
Allegations: In July of 2022, Ms. Michelle Kaefer filed a lawsuit on behalf of her deceased mom against Oakmont Center for Nursing & Rehabilitation. During the two months Ms. Rosie Mae Sepelyak resided at the facility, she was neglected by staff. On August 31st of 2021, Ms. Sepelyak was discovered unresponsive in her room. She was taken to the hospital where she was diagnosed with a septic UTI and parotitis. A urinalysis found E. Coli in her urine. Ms. Sepelyak was admitted to the hospice unit of the hospital where she passed on September 5th of 2021. Her death certificate listed urosepsis as one of the primary causes of death.
Edwards, Jack D. E/O vs. Oakmont Center for Nursing & Rehabilitation
Allegations: Ms. Barbara Edwards, the surviving spouse of Mr. Jack Edwards, filed a nursing home negligence lawsuit against Oakmont Center for Nursing & Rehabilitation. Due to Mr. Edwards limited mobility, he was wheelchair-bound and had to be assisted out of bed with a Hoyer lift. During a transfer out of bed, Mr. Edwards’ left leg became caught between the Hoyer lift and his wheelchair. Due to this incident, Mr. Edwards suffered a hematoma on the leg, pain, and necrosis. He underwent several surgeries to treat the necrosis.
Deficiencies at Oakmont Center for Nursing & Rehabilitation for Resident Care Violations
July 30th, 2021, Inspection at Oakmont Center for Nursing & Rehabilitation
Based on review of the facility’s policies and interviews with staff and residents, inspectors determined that the facility had failed to provide adequate supervision to prevent accidents. A resident suffering from dementia was able to access hot water in the water dispenser outside her room. According to the facility’s policies, all hot water faucets are supposed to be deactivated in the water dispensers to avoid accidents. The resident suffered from a burn and blister from the scalding water.
July 15th, 2021, Inspection at Oakmont Center for Nursing & Rehabilitation
An inspection conducted at Oakmont Center for Nursing & Rehabilitation found that the facility had failed to report an allegation of neglect to the State. During an interview with residents, inspectors were told that an aide had refused to change multiple residents’ diapers and provide care to them. When residents tried to ask the aide for care, she would verbally abuse them. This allegation should have been reported to the State when the Nursing Home Administrator was made aware of the residents’ complaints.
