Tanner, Eleanor A. E/O vs. Life Care Center of Orange Park
Allegations: During her time as a resident at Life Care Center of Orange Park, Ms. Tanner was not provided with adequate care. Ms. Tanner’s health was neglected, which led to her developing and suffering from infections such as sepsis and respiratory failure. After Ms. Tanner’s untimely death, her surviving children pursued a nursing home negligence lawsuit against the facility.
Bruce, Rosa C. E/O vs. Life Care Center of Orange Park
Allegations: The estate of Ms. Bruce sued Life Care Center of Orange Park for a wrongful death claim. According to the lawsuit’s complaint, Ms. Bruce was a “two-person assist,” meaning that two staff members were required to transfer her. Ms. Bruce was improperly transferred, which resulted in a fractured right femur. Furthermore, Ms. Bruce’s physician was not notified of the femoral fracture and unfortunately, Ms. Bruce was not given the necessary medical treatment for her injury in time.
Maroney, Boyd E. E/O vs. Life Care Center of Orange Park
Allegations: Mr. Maroney suffered from neglect and the failure of Life Care Center of Orange Park to provide him with appropriate medical care while he was a resident in their facility. Mr. Maroney developed severe constipation, fecal impaction, and abdominal distention. His conditions were left untreated, which led to a perforated bowel and sepsis infection. Mr. Maroney passed away as a result of septic shock. The personal representative of his estate sought legal action via a wrongful death case against Life Care Center of Orange Park on behalf of Mr. Maroney.
Deficiencies at Life Care Center of Orange Park for Resident Care Violations
July 29th, 2021, Inspection at Life Care Center of Orange Park
During this inspection at the facility, inspectors observed a resident with a foley catheter. The resident’s catheter bag was on the floor next to her bed and had dark brown sediment inside of it. Upon review of the resident’s medical records, it was revealed that she was suffering from a UTI (urinary tract infection). Per the facility’s own policies and procedures, catheter bags should never be placed directly on the floor, as it can increase the likelihood of bacteria getting into the bag and causing an infection.
April 19th, 2018, Inspection at Life Care Center of Orange Park
While reviewing a resident’s medical records during an inspection at the facility in April of 2018, inspectors noticed a discrepancy. Resident #68 was seen by the facility’s physician who ordered a “STAT” urinalysis for her. “STAT” tests, per Life Care’s own policies and procedures, are supposed to be done as soon as possible and take precedent over all other pending blood work and urine tests. Resident #68’s urine was not collected for testing until several days after the facility’s physician ordered the “STAT” test, thus delaying her medical care. Once her urinalysis results came back, it was determined that she was suffering from a UTI (urinary tract infection) with E. Coli.
