March 7th, 2023, Inspection at Warren Barr South Loop
During an inspection at Warren Barr South Loop, inspectors observed an unsupervised resident try to get out of his wheelchair. A registered nurse (RN) was able to get to the resident in time and sit him back down. Inspectors overheard the RN telling other staff that the resident is a fall-risk and cannot be left alone. Inspectors also observed that the resident’s chair alarm (an alarm that sounds when a resident tries to get up from the wheelchair) was not working.
January 26th, 2023, Inspection at Warren Barr South Loop
This inspection revealed that Warren Barr South Loop had failed to prevent residents from developing bedsores, also called pressure ulcers. Furthermore, staff were incorrectly using the Low Air Loss Mattress (LALM). Air mattresses are used to alleviate pressure and prevent wounds from deteriorating. However, staff at Warren Barr South Loop had not adjusted six residents’ LALMs to accommodate their weights.
December 2nd, 2022, Inspection at Warren Barr South Loop
Based on observation, record review, and interviews, inspectors determined that staff at Warren Barr South Loop failed to prevent a resident’s bedsore from deteriorating. The resident had to be transferred to the ER for treatment of a stage three bedsore and sepsis infection.
