According to the complaint, staff at the facility failed to prevent Mr. Hill from suffering from severe dehydration and unexpected injuries. The facility also failed to timely transport Mr. Hill to the hospital to receive medical treatment.
Collazo, Rosa E/O vs. Guardian Care Nursing & Rehabilitation Center
Allegations: In March 2023, Ms. Collazo’s estate filed a wrongful death case against Guardian Care Nursing & Rehabilitation Center. Ms. Collazo’s surviving husband alleges that Ms. Collazo died as a direct result of the nursing home’s negligent care.
Paul, William E/O vs. Guardian Care Nursing & Rehabilitation Center
Allegations: While at Guardian Care Nursing & Rehabilitation Center, Mr. William Paul suffered from negligent care resulting in injury and death. Mr. Paul’s son, Mr. James Paul, filed a wrongful death lawsuit against Guardian Care, Inc. in January 2023.
Evans, Leotis vs. Guardian Care Nursing & Rehabilitation Center
Allegations: In 2018, Mr. Leotis Evan’s legal guardian filed suit against Guardian Care, Inc. after Mr. Evans was subjected to negligence in the facility. During his time in the nursing home, Mr. Evans suffered from the development and deterioration of bedsores, also known as pressure ulcers or decubitus ulcers, malnutrition, and falls.

February 17th, 2022, Inspection at Guardian Care Nursing & Rehabilitation Center
During a standard health inspection at the nursing home, inspectors were informed that Resident #38 had fallen and suffered a left femur fracture on 11/22/21. While reviewing the resident’s records, it was noted that staff had inaccurately put that when the resident had returned to the facility from the hospital after the fracture, the resident had returned from another nursing home, excluding the hospital discharge from the notes.
