Sandra Hoover filed the suit on behalf of the Estate of Gussie Gechtman against Boulevard nursing home. The allegation involves negligence on the part of Boulevard Rehab in terms of the provision of sufficient care to Sandra Hoover. The facility allegedly failed to take action to prevent falls of Gussie Gechtman within the facility which resulted to physical injuries and pain of the plaintiff.

Carol Decicco filed a suit for the Estate of Rose Bocchimuzzo against Boulevard Rehabilitation Center and Southern Healthcare Management, LLC. The Complaint alleges as follows:
Rose Bocchimuzzo was admitted as a resident in Boulevard Rehabilitation Center on July 13, 2010 and initially had zero pressure sores. On July 15, 2010, she was reported to have a red circular spot on her coccyx. Her skin continued to break down and developed painful pressure sores and on July 20, 2010, she was discharge because her family was unhappy with the deterioration she experienced. The suit involves an alleged failure on the part of Boulevard Rehabilitation Center to provide proper and adequate medical intervention to the condition that Ms. Bocchimuzzo experienced.

Helen Anisman filed a lawsuit against Boulevard Rehabilitation Center. Allegedly, the facility failed to provide Ms. Anisman proper care that is consistent with the instructions of the plaintiff’s physician. The alleged failure of the facility resulted to permanent physical injury, disability, disfigurement, mental anguish and loss of capacity to enjoy life.
Recent Health Inspection Citations Against Boulevard Rehabilitation Center
For the past years, Boulevard Rehabilitation Center had a number of citations related to fire life safety. The latest of these surveys was on September 19, 2017 and there was one particular citation against Boulevard Rehabilitation Center.
September 19, 2017 Fire/Life/Safety Inspection Report
On September 19, 2017, an unannounced fire life safety recertification survey was conducted at Boulevard Rehabilitation Center and it was found out that the facility is not in compliance with Code of Federal Regulations (CFR) 42, Section 483 Subpart B, 42 CFR 483.307 and the National Fire Protection Associate (NFPA) 101 (2012) requirements for nursing homes.
The non-compliance involves failure to maintain the building with cooking suppression system which can affect all smoke compartments, staff, visitors and all residents. Other non-compliance involves failure to maintain the building fire alarm system, failure to maintain building corridor door opening assemblies and failure to maintain building exit egress. These issues were remedied after the survey results were released.
