Deficiencies at Brightwood Center for Resident Care Violations
May 5th, 2021, Inspection at Brightwood Center
During an inspection at the nursing home, surveyors found a resident in bed with a completely full foley catheter bag. Upon conversing with the resident, the resident explained that they had the foley catheter to protect further skin breakdown on their butt and coccyx. However, staff at the Brightwood Center had failed to empty the catheter bag on two occasions in the last month which caused the urine to back up into the foley tube and soak the bed. The point of the foley catheter was to keep the resident’s bottom and back dry to prevent any moisture that could facilitate skin breakdown.
January 9th, 2020, Inspection at Brightwood Center
Based on observation, record review, and staff interviews, it was determined that Brightwood Center failed to completed accurate assessments of a resident’s condition. A progress note of Resident #30’s oral exam dated 11/20/18 recorded that he had numerous decayed and missing teeth. There is then a significant change when his next assessment is recorded as having no decayed or broken/missing teeth on 8/7/19. Resident #30’s next oral assessment dated 9/25/19 stated that all of his remaining teeth are decaying and most of his teeth are missing. During an interview with Brightwood Center’s Nurse Executive, it was acknowledged that Resident #30’s assessment on 8/7/19 was inaccurate.
November 15th, 2018, Inspection at Brightwood Center
While inspecting the nursing home, surveyors found that a resident’s PICC line dressing had not been changed for 12 days. According to the facility’s Central Vascular Access Device (CVAD) Dressing Change Policy, which was provided to surveyors by the Brightwood Center’s Director of Nursing, sterile dressing changes on PICC lines should occur at least weekly if the integrity of the dressing has not been compromised (soiled, wet, or loose). If sterile dressing changes are not provided in a timely manner, it can lead to the increased risk of infections.
