ESTATE OF FORREST RAY NEAL JR vs. SUNRISE OF JACKSONVILLE & SUNRISE SENIOR LIVING
Allegations: Mr. Ray was admitted to Sunrise of Jacksonville assisted living on 3/26/2017. Sunrise of Jacksonville failed to provide a safe environment and failed to prevent Forrest Ray Neal Jr. from falling in the ALF. As a result of his injuries at Sunrise of Jacksonville, Mr. Ray wrongfully died.
ESTATE OF VERA SYKES vs. SUNRISE OF JACKSONVILLE & SUNRISE SENIOR LIVING
Allegations: Ms. Sykes was pushed to the ground by another resident of Sunrise of Jacksonville. This incident occurred on January 19, 2022 and the attacker had a documented history of violent behavior. She immediately reported hip pain and arm pain following the resident on resident attack. She was taken to Orange Park Medical Center and diagnosed with a intraparenchymal hematoma on the right side of her brain, left hip fracture, left wrist fracture, and decline in mobility. Ms. Sykes died on May 1, 2022.
Fines Imposed Against Sunrise of Jax Assisted Living Facility
Sunrise of Jacksonville was fined on two occasions by the State of Florida Agency for Health Care Administration. The first fine was imposed on May 19, 2017 for untimely submission of an adverse incident that should have been reported to the Agency. The second fine was imposed on March 8, 2018 for failure to acquire criminal background screening for personnel.
Deficiencies at Sunrise of Jacksonville for Resident Care Violations
A July 21, 2014 Inspection at Sunrise of Jacksonville revealed that the facility was cited for failure to ensure that a nurse was available to take or supervise the taking of vital signs during the medication pass process. The facility was also cited for failure to provide a safe living environment for a resident when her cannulas were found to be unsecured. Furthermore, the facility was cited for failure to provide qualified staff to meet the needs of residents reviewed.
A December 22, 2015 Inspection at Sunrise of Jacksonville revealed the facility failed to obtain vital signs by a licensed nurse for medication requiring parameters of a resident. The record showed that the nurse did not record all the vital signs needed before giving the resident his medication. The nurse also confirmed the staff has not yet been given education on the taking of vital signs and the care manager was not a certified nursing assistant.
A July 13, 2020 Inspection at Sunrise of Jacksonville revealed the facility failed to ensure the safety of its residents by failing to report the following information: facility census, available beds, inventory, needs, and other daily related information for COVID-19.
