Background Information on Crestview Rehabilitation Center
Crestview Rehabilitation Center is a for-profit 180-bed nursing home located at 1849 First Avenue East, Crestview, FL 32539. Crestview has an overall rating of five out of five stars, based on Medicare.gov data from September 2024 and earlier. There have been prior lawsuits against Crestview Rehabilitation Center as well as inspection citations and deficiencies for violations in resident care.
Deficiencies at Crestview Rehabilitation Center for Resident Care Violations
An October 1, 2020 Inspection at Crestview Rehabilitation Center revealed that the facility failed to ensure that expired drugs and biologicals were removed from the medication cart. The inspection also revealed that the facility failed to perform proper hand hygiene during wound care. Not having the correct hygiene processes in place during wound care (such as bed sores) can lead to a possible infection or even worse, sepsis. The Department of Health and Human Services noted a potential for actual harm resulting from these deficiencies.
A July 8, 2020 Inspection at Crestview Rehabilitation Center revealed that the facility did not provide notice of transfer or discharge for residents. Based on an interview of these residents, the inspection revealed that the facility failed to provide discharge information to ensure a safe and orderly transition for these individuals.
A March 14, 2019 Inspection at Crestview Rehabilitation Center revealed that the facility failed to provide appropriate care and services in accordance with resident’s plan of care. Furthermore, during two kitchen inspections, issues were discovered regarding the facility’s Quality Assurance Performance.
A March 2, 2023 inspection at Crestview Rehab resulted in citations for 483.25 Quality of Care violations, as well as 483.25(c) loss of mobility violations.
A May 2024 inspection revealed no violations. Due to recent improvement in survey results, Crestview Rehab Center has become a 5 star rated facility on Medicare’s Care Compare.
