June 1st, 2023, Inspection at Glendora Grand During an inspection at Glendora Grand in June 2023, it was determined that facility staff failed to follow standard infection prevention and control procedures. Several staff members failed to wash their hands and use protective equipment while caring for residents with infectious diseases. March 30th, 2023, Inspection at Glendora Grand A complaint inspection occurred at the facility following an incident between Resident #1 and CNA #1. Based on interviews, it was determined that Resident #1 and CNA #1 were in a verbal altercation which led to CNA #1 cursing at the resident and kicking his wheelchair. December 14th, 2022, Inspection at Glendora Grand This December 2022 inspection at Glendora Grand revealed that staff had failed to provide a resident with the appropriate treatment and care prescribed by a physician. The resident had a history of fecal impactions and constipation. Therefore, the resident was prescribed two stool softeners by the facility’s physician to be taken daily. Staff were instructed to monitor the resident’s bowel sounds and bowel movements every shift. However, staff failed to follow the physician’s orders for medication and monitoring. This led to the resident requiring hospitalization for a fecal impaction and abdominal distension. Unfortunately, due to the delay in treatment, the resident suffered from bowel perforation and septic shock, causing his death. November 30th, 2022, Inspection at Glendora Grand A complaint inspection occurred at Glendora Grand in November 2022 following an altercation between two residents. According to interviews, Resident #2 hit Resident #1 on the head while they were out on the facility’s patio. Inspectors found that the facility’s staff had failed to protect Resident #1 from suffering physical abuse.Disclaimer*Each case is different.
