Allegations: The administratrix of Ms. Messer’s estate, Amie Chaufin, sued the Logan Center after Ms. Messer’s untimely passing. While she was in the facility for rehabilitation, Ms. Messer suffered from dehydration, malnutrition, and a sepsis infection.
Deficiencies at Logan Center for Nursing Home Resident Care Violations
April 6th, 2022, Inspection at Logan Center
During an inspection at the facility, inspectors discovered that the facility had failed to ensure residents received treatment and care according to nursing standards. A diabetic resident was supposed to have their blood glucose tested by fingerstick two times a day. However, review of the resident’s medical records showed that staff had not been doing so for several days. Blood glucose monitoring is a very important practice for diabetic patients.
September 26th, 2019, Inspection at Logan Center
Based on observations and medical record review, it was determined that the Logan Center had failed to provide and implement an infection prevention and control program. Resident #28 was diagnosed with having Staphylococcus aureus, Extended spectrum beta-lactamases (ESBL), pneumonia, and Vancomycin-resistant Enterococcus (VRE). The resident also had multiple bedsores. The facility failed to have personal protective equipment (PPE) available for staff, such as masks and gloves. There were also no signs placed on Resident #28’s door to alert staff or visitors of the infection precautions needed upon entering the room. According to The Centers for Medicare & Medicaid Services (CMS), these failed practices had the potential to affect more than a limited number of residents at risk for serious harm and/or death.
September 19th, 2018, Inspection at Logan Center
During an interview with Resident #51, inspectors were told that the resident had problems with getting staff to answer her call light. Furthermore, she said that she had been cursed at by one certified nursing assistant (CNA) and threatened by another.
