Allegations: The facility failed to follow doctor’s orders regarding feeding, which led to Mr. Fedderson aspirating. The aspiration resulted in acute respiratory failure, which led to Mr. Fedderson’s wrongful death.
Davis, Billy vs. PruittHealth Santa Rosa
Allegations: Staff at PruittHealth – Santa Rosa provided Mr. Davis with improper and unsafe physical therapy, which resulted in a neck injury and hospitalization.

Multiple residents reported abuse by a certified nursing assistant (CNA) to the Florida Department of Children and Families (DCF). The CNA was reported as being verbally abusive and using profanities at residents, as well as being physically rough with residents.
March 6, 2017, Inspection at PruittHealth – Santa Rosa
During an inspection at the facility, it was determined that staff members had failed to implement the appropriate fall prevention methods for residents who were labeled as fall risks. Due to these failures, multiple residents suffered falls, with some resulting in injuries.
