PruittHealth – Santa Rosa is a for-profit 120-bed nursing home in Milton, FL. Located at 5530 Northrop Road, this nursing home is just across the street from Santa Rosa Medical Center. At the time of this post, Medicare has given PruittHealth – Santa Rosa, LLC an overall rating of two out of five stars on their Care Compare site. In September 2021, PruittHealth – Santa Rosa was fined $20,000 by the Agency for Health Care Administration (AHCA) for failing to prevent the elopement of two residents from the facility. Allegations: During her residency at PruittHealth – Santa Rosa, Mrs. McGlone suffered the development of bed sores due to improper offloading. The wound became septic due to improper wound care, which ultimately led to Mrs. McGlone’s untimely passing.
Devault, Clara E/O vs. Pruitt Health Santa Rosa Nursing Home
Allegations: As a result of the facility’s negligence, Ms. Devault suffered the development of bed sores, severe malnutrition, and dehydration. Ms. Devault passed soon after she was taken to the hospital from the nursing home.
Sukup, Linda E/O vs. PruittHealth – Santa Rosa
Allegations: PruittHealth – Santa Rosa failed to monitor and prevent severe fluid retention and weight gain in Mrs. Sukup. As a result of the facility’s negligence, Mrs. Sukup suffered pain and suffering, physical disfigurement, hospitalization, and wrongful death.
Fedderson, Raymond E/O vs. PruittHealth Santa Rosa in Milton, FL
Allegations: The facility failed to follow doctor’s orders regarding feeding, which led to Mr. Fedderson aspirating. The aspiration resulted in acute respiratory failure, which led to Mr. Fedderson’s wrongful death.
Davis, Billy vs. PruittHealth Santa Rosa
Allegations: Staff at PruittHealth – Santa Rosa provided Mr. Davis with improper and unsafe physical therapy, which resulted in a neck injury and hospitalization.
Deficiencies at PruittHealth Santa Rosa for Resident Care Violations
As of the date of this post, Pruitt Health Santa Rosa received a 1 star rating (the lowest rating possible) by Medicare on Health Inspections. Medicare calls this one star rating, “much below average.”
Multiple residents reported abuse by a certified nursing assistant (CNA) to the Florida Department of Children and Families (DCF). The CNA was reported as being verbally abusive and using profanities at residents, as well as being physically rough with residents.
During an inspection at the facility, it was determined that staff members had failed to implement the appropriate fall prevention methods for residents who were labeled as fall risks. Due to these failures, multiple residents suffered falls, with some resulting in injuries.
