Resident #47 explained to surveyors that while at home, she liked having her teeth brushed 3-5 times a day. Resident #47’s care plan indicated that she required extensive assistance for personal hygiene. Upon review of Resident #47’s record, there was no indication that oral hygiene care had been offered to her.

Based on observations and record review, it was determined that staff at the facility failed to provide appropriate catheter care for a resident. Resident #32, who was suffering from a UTI at the time of this inspection, was observed multiple times by surveyors with his catheter bag crumpled up, at or above the level of his bladder, with the tubing kinked. Catheter bags should be kept below the level of the bladder and free of kinks to prevent bacteria from traveling into the bladder.
