January 12th, 2023, Inspection at Rehabilitation and Healthcare Center of Tampa
This inspection at the facility uncovered that staff had failed to ensure residents received proper care and treatment regarding urinary tract infections (UTIs). Based on interviews and record review, it was determined that Resident #2 had a 72-hour delay between when abnormal lab results came back and when a physician was notified of the results. Furthermore, Resident #20 had a 48-hour delay in obtaining a physician-ordered urine specimen.
November 15th, 2022, Inspection at Rehabilitation and Healthcare Center of Tampa
During an interview with a resident in the Rehabilitation and Healthcare Center of Tampa, Resident #47, surveyors were told that she had received no oral care while in the facility. Resident #47 explained to surveyors that while at home, she liked having her teeth brushed 3-5 times a day. Resident #47’s care plan indicated that she required extensive assistance for personal hygiene. Upon review of Resident #47’s record, there was no indication that oral hygiene care had been offered to her.
May 7th, 2021, Inspection at Rehabilitation and Healthcare Center of Tampa
Based on observations and record review, it was determined that staff at the facility failed to provide appropriate catheter care for a resident. Resident #32, who was suffering from a UTI at the time of this inspection, was observed multiple times by surveyors with his catheter bag crumpled up, at or above the level of his bladder, with the tubing kinked. Catheter bags should be kept below the level of the bladder and free of kinks to prevent bacteria from traveling into the bladder.
