How are Illinois Nursing Homes Rated?
Nursing homes across the United States undergo frequent evaluations by the Centers for Medicare & Medicaid Services (CMS).
All Illinois facilities are examined by CMS to determine their ratings, which are between 1 to 5 stars:
A 1-Star facility is considered to be much below average;
2-Stars represent below average;
3-Stars represent average;
4-Stars are above average, and
5-Stars are much above average.
This rating system allows the public to easily compare facilities when choosing care for themselves or a loved one. Health, complaint, and infection inspections, staffing numbers – including staff turnover rates and whether or not a facility is short staffed – and quality measures are all considered by CMS before assigning a star rating to a facility.
Quality measures refer to the nursing home’s performance in certain areas of care. For instance, CMS considers the percentage of residents who have lost weight, the frequency of hospitalizations, and the percentage of pressure ulcers or falls among residents, and more. Along with their star rating, CMS also indicates whether or not a facility has been recently cited for resident harm from abuse or neglect. If you are looking at Medicare’s Care Compare online tool, facilities with the above issues will have a red hand icon next to their names.
Illinois Nursing Home Statistics
Across the state of Illinois, there are currently 680 nursing homes in operation. 607 of those facilities have been cited in the past for infection-related deficiencies, and 265 of them have been cited for serious deficiencies – issues that put residents’ health or safety in immediate danger. Together, Illinois nursing homes have been issued $75.1 million in fines as the result of cited deficiencies.
As of January 2025, the Illinois facility with the highest number of deficiencies and the highest total of fines is Chicago Ridge SNF. Chicago Ridge SNF has amassed a total of 143 deficiencies and has been fined a total of $986,612. The facility with the most recent serious deficiencies is The Pearl of Elk Grove. In late September of 2024, they were cited for the failure to protect residents from all types of abuse and the failure to respond to alleged violations. The state of Illinois, as of January 2025, ranks 50th in the nation for nursing home quality. The average CMS star rating for Illinois facilities is 2.59 out of 5 stars. Additionally, approximately 8.45% of residents in the state have been documented as developing pressure ulcers.

1-Star Nursing Homes in Illinois
As of January 2025, roughly 38.5% of Illinois’ nursing homes are rated 1-Star. 77 of the 260 nursing homes rated 1-Star have been cited for abuse by Medicare. On the other hand, 0 of the 91 Illinois facilities rated 5-Star have been cited for abuse. While the star ratings should be deeply considered when choosing facility placements, they are not able to accurately predict whether or not your loved ones will experience poor care. It is certainly possible for a resident to be abused at a 5-Star facility, but the likelihood is lower.

That means that roughly 28% of the facilities listed below have been cited for abuse and/or neglect, whereas the percentage for 1-Star facilities is slightly higher at 30%. While having a loved one or yourself in a 2-Star facility does not at all guarantee that abuse will occur, the statistical likelihood of it decreases with facilities with higher star ratings.
