Being the worst rated nursing home in New York is a dubious title. So how are New York’s worst rated nursing homes identified?
The Federal Government, through CMS, has an objective 5 star rating system which awards nursing homes 1 through 5 stars based on quality. A 5 star rating is the highest. A 1 star rating is the worst.
5 star nursing homes are relatively common nationwide. 1 star facilities are rare. Shockingly, the state of New York has 107 1-star nursing homes. By comparison, Delaware only has 5 1-star nursing homes.

How Does CMS Determine NY Nursing Home Quality Through Star Ratings?
CMS announced in March that they recently updated their 5 star rating system. CMS comes to their rating based on various factors, including health inspections, resident injuries, hospital admissions, staffing levels and quality measures. The recent update to Medicare star rating system includes a deeper dive into the understaffing analysis inside nursing homes.
Why Are New York Nursing Homes So Bad?
The short answer is understaffing. When there are not enough staff to care for high needs patients, the result is preventable injuries.
Common Injuries Due to New York Nursing Home Negligence
- Bed sores and pressure injury wounds.
- Falls.
- Infections.
- Unexplained traumatic injuries, like fractures and brain bleeds.
- Weight loss of more than 10% of BMI.
- Wrongful death.
Why are New York Nursing Homes Understaffed and Short Staffed?
The short answer is money. Most nursing homes are private, for-profit companies. In order to make the most amount of money, they need to spend as little of it as possible. When we obtain a nursing home’s financial books, universally, the largest expense on the budget is staffing. If you have less workers caring for patients, the corporate owners make more money.
