Avante at Boca Raton was recently added to AHCA’s nursing home watchlist. The Agency for Healthcare Administration (AHCA) has added Avante at Boca Raton to its nursing home watchlist due to repeated citations after state surveys and inspections. The 144 bed nursing home based in Boca Raton, Florida has been added after receiving citations and violations by the agency.

Avante at Boca Raton was cited for deficiencies and fined on the following dates:
May 18 of 2016,
February 5 of 2016, and,
November 29 of 2015.
