Smith, Isabelle C. vs. Dunbar Center
Allegations: During her residency at Dunbar Center, Isabelle C. Smith alleges that she suffered substantial neglect and lack of medical care. Ms. Smith’s surgical wound was not properly cared for, which resulted in infection of the surgical site. Due to these failures, she had to undergo two surgical procedures to treat the infected wound. Ms. Smith later sued Dunbar Center for nursing home negligence.
Deficiencies at Dunbar Center for Resident Care Violations
March 21st, 2024, Inspection at Dunbar Center
During an inspection at Dunbar Center in March of 2024, surveyors found that the facility had failed to provide appropriate wound care for three residents with pressure ulcers. Each of the residents had orders in their charts for staff to turn or reposition them in bed every 1-2 hours to prevent pressure buildup. These orders were not being followed by staff during this inspection, contributing to preventable skin breakdown like bed sores.
July 6th, 2023, Inspection at Dunbar Center
An inspection at the facility in July of 2023 resulted in Dunbar Center being cited for failing to use appropriate infection control measures, such as wearing PPE (personal protective equipment) around residents suffering from infections.
February 15th, 2023, Inspection at Dunbar Center
In February of 2023, inspectors found that a resident at the facility had suffered a major fall resulting in fractures to the C1, C2, C4, and C6 vertebrae, as well as a laceration to the scalp. The facility had been made aware of the resident’s high risk of falls and muscle weakness but had failed to provide appropriate fall prevention measures. Additionally, even after the fall, the facility failed to update the resident’s care plan to add new fall interventions.
August 3rd, 2022, Inspection at Dunbar Center
During this inspection at Dunbar Center, it was found that facility staff had been regularly mismanaging several residents’ medications. Inspectors determined that staff had not been following doctors’ instructions, such as administering a specific dosage or administering medications on a schedule (one pill every hour, etc.) while administering patients’ medications.
