Allegations: This case involved allegations against Tuskawilla Nursing and Rehab Center for inadequate healthcare and supervision. The resident had a severe infection and severe wound deterioration to the extent that he required additional surgeries. The resident, and his wife, suffered as a result of the facility’s failures.
Eldridge v. Sovereign Healthcare of Tuskawilla, LLC d/b/a Tuskawilla Nursing and Rehab Center
Allegations: This case involved allegations against Tuskawilla Nursing and Rehab Center for failure to provide sufficient staff to meet the resident’s needs and failure to provide adequate care. During his time at the facility, the resident developed a Stage IV pressure ulcer. The resident suffered physical pain and injury, and his untimely death, as a result of this injury.
Cruz v. Sovereign Healthcare of Tuskawilla, LLC d/b/a Tuskawilla Nursing and Rehab Center
Allegations: This case involved allegations against Tuskawilla Nursing and Rehab Center for failure to provide adequate care and support services to prevent a resident from falling. The resident fell, while at the facility, causing a subdural hematoma. As a consequence of this injury, the resident died.
Fines Imposed Against Tuskawilla Nursing and Rehab Center
Tuskawilla Nursing and Rehab Center was fined on December 13, 2019 by the Agency for Health Care Administration for failure to provide adequate and appropriate healthcare to a resident and for failure to be in compliance with the survey criteria. The facility was subjected to an assignment of conditional licensure status and required to pay the Agency for Health Care Administration $16,000.00. Tuskawilla Nursing and Rehab Center was also fined on July 7, 2016 by the Agency for Health Care Administration for failure to comply with the Care Provider Background Screening process. The facility was required to pay the Agency for Health Care Administration $500.00.
Deficiencies at Tuskawilla Nursing and Rehab Center for Resident Care Violations
- A September 1, 2020 Inspection at Tuskawilla Nursing and Rehab Center revealed the facility did not maintain complete medical records that reflected changes in condition and changes in physicians’ orders. The above-mentioned medical records did not contain any evidence that the physician, nursing, or other staffing notes were ever changed or updated to include the residents change in condition.
- A December 19, 2019 Inspection at Tuskawilla Nursing and Rehab Center revealed the facility failed to honor resident’s preference for daily routine activities and failed to provide adequate care and services. The facility did not inquire into the resident’s preferences and routines and failed to incorporate this into the care planning process. The facility also did not monitor, treat, and promote healing for a change in skin condition of a resident.
- An August 2, 2019 Inspection at Tuskawilla Nursing and Rehab Center revealed the facility failed to ensure that there were sufficient staff on a unit at a designated time and the facility failed to ensure timely administration of medication. The Department of Health and Human Services noted a potential for actual harm resulting from these failures.
