Valley Center, a 130-bed nursing home owned by Genesis HealthCare, can be found at 1000 Lincoln Drive in South Charleston, West Virginia. Genesis HealthCare is one of the country’s largest inpatient healthcare providers. They own and operate nursing homes, rehabilitation centers, and assisted living facilities in more than thirty states.
As of this post, Valley Center remains unrated on Medicare’s Care Compare site. According to Medicare, Valley Center is not rated due to a history of serious quality issues. Valley Center is currently part of the 2023 Special Focus Facilities (SFF) program. The SFF program identifies nursing homes and other care facilities that have a record of poor inspection results. Blakenship, Georgia E/O vs. Valley Center
Allegations: Shannon Egnor, the niece and executrix of the estate of Ms. Blankenship, filed a nursing home abuse and neglect case against Valley Center in early 2020. According to the complaint, Ms. Blankenship suffered from numerous falls, medication errors, and a fecal impaction while under the care of Valley Center’s staff. The neglect she suffered in the facility allegedly contributed to her untimely passing.
Sloan, Willia E/O vs. Valley Center
Allegations: Gary Sloan, the son and personal representative of the estate of Ms. Sloan, sued Valley Center after his mother’s wrongful death. According to the complaint, while Ms. Sloan was a resident at Valley Center, she was subject to abuse and neglect causing significant injuries. Mr. Sloan alleges that Valley Center failed to provide his mother with a safe environment and quality care.
Deficiencies at Valley Center for Resident Care Violations
April 25th, 2023, Inspection at Valley Center
A complaint inspection was conducted at Valley Center in April 2023 following the death of a resident, Resident #130. Resident #130 was found unresponsive in her room after acting lethargic the day prior. Resident #130 received CPR but could not be revived. According to the facility’s records, Resident #130’s daughter had visited her the day prior. Staff witnessed a conversation between Resident #130 and her daughter about the daughter bringing in, “something for pain, the good stuff.” Staff did not report this conversation. Resident #130 passed away after suffering an opioid overdose. In addition, Valley Center was cited for not reporting the incident to the proper authorities.
March 3rd, 2022, Inspection at Valley Center
Based on an interview with Resident #3, it was determined that Valley Center failed to provide the resident with necessary wound care. While speaking with Resident #3, inspectors were told that she had recently had a skin graft on her left thigh. The doctor’s orders were to keep the wound uncovered so that it could properly drain. A nurse and a certified nursing assistant (CNA) failed to follow these orders and wrapped a pillowcase around Resident #3’s left thigh. When they removed the pillowcase hours later, it ripped the scabs off of the wound, causing immense bleeding and damage to the healing wound.
